Can’t you see that is a Communist hospital facility where the prone man is waiting for medical attention? The chancletero physician is approaching him to provide assistance. The prone man has just turned over on his comfortable pillow after relieving himself. Note the puddle on urine located at his former position. This excellent health care is free in socialist Cuba.

Your are all mistaken. Because of the inhumane US blockade there are very few goods in Cuba. This gentleman is simply the first in line (en la cola) ensuring that he gets his government allowed rations.
